Bristol Hospice programs are designed to promote quality and comprehensive services to our patients, families and communities. Bristol Hopsice was formed in 2006, focusing on the delivery of hospice services. Our Bristol Hospice programs provide hospice services to meet the physical, psychosocial, and spiritual needs of our patients and their families/caregivers. An interdisciplinary team of professionals and volunteers develop an individualized plan of care which includes, as appropriate, the following services: nursing, physicians, hospice care aides, counseling, spiritual support, therapy, dietary, volunteers, durable medical equipment, supplies and bereeavement services.
